If you are a patient

Therapy Contract

Clinician

Dr Dean Whybrow is a clinical nurse specialist in psychological therapies. He has significant experience in offering both Cognitive Behavioural Therapy (CBT) and Eye Movement Desensitization and Reprocessing (EMDR) to people aged over 18. He is registered as a mental health nurse by the Nursing and Midwifery Council, accredited as a cognitive behavioural therapist by the British Association of Behavioural & Cognitive Psychotherapies, and accredited as an EMDR Practioner by the EMDR Association.

Length of sessions

CBT: 50 minutes

EMDR: 50 minutes/80 minutes

Fees, and how and when these should be paid

Self-funding: Up to 50 minutes - £90. Up to 80 minutes - £135. Payable immediately after the session via BACs bank transfer or PayPal

Insurance company: Fees are as agreed with and payable by the insurer; Any policy excess is payable by you (the patient)

Referring agencies: Fees are as agreed with and payable by the referring agency

Cancellation policy and any related payments

Should you fail to attend an appointment, you will be charged for the full cost unless you provide 24 hours notice.

What happens if you are suddenly unable to attend a therapy session

Sometimes, an event outside of your control (e.g. illness, family emergency) may lead to a last-minute cancellation. The first time this happens, this will not be counted as a non-attended appointment. If it becomes a regular occurrence, then it will be counted as a short notice cancellation, and you will be charged for the full cost unless you provide 24 hours notice.

How you will be contacted between sessions

Communication between sessions, if necessary, will be via phone, email, and/or text message. 

How you can make contact between sessions

Should you need to get in touch between sessions, then please use the phone, email, and/or text message. Please note: Evenkeel is unable to provide a crisis response service. If you have an urgent concern, are at risk or are in crisis, then please contact your GP, GP out-of-hours service, The Samaritans, or emergency services or visit A&E.

Where sessions will be held

Unless otherwise agreed, sessions will be online: Therapy is provided via a video conferencing platform using Microsoft TEAMS. If TEAMS is not working, then we may be able to switch to the telephone for that specific session but then return to TEAMS at the next session. 

If Face-to-Face sessions have been agreed upon, then the location is: All Ears Centre, 27/28 Blue Street, First Floor, Carmarthen SA31 3LE

When sessions will be held

The exact date and time of sessions will be negotiated either at regular intervals or at each session. Sessions are typically available on a weekday evening after 1700 or on a Saturday starting at 0800.

Risk of harm

Evenkeel is unable to provide a crisis response service. If you have an urgent concern, are at risk or are in crisis, then please contact your GP, GP out-of-hours service, The Samaritans, or emergency services or visit A&E. We may share information without your consent if we are concerned that you are at risk of harm or there is a risk of harm to others. This could include but is not limited to, your GP, emergency services, the police, and/or social services.

Raising a concern

If you have a concern about the service provided, then please do not hesitate to let me know. I will endeavour to resolve your concern as far as I possibly can. You can also contact my statutory regulator, which is the Nursing & Midwifery Council (NMC).  Alternatively, you contact the British Association of Behavioural & Cognitive Psychotherapies (BABCP) or the EMDR Association UK (EMDR UK).

Clinical Will

If I am no longer able to provide therapy, for example, due to my serious illness or death, I have identified a third person (our Data Protection Officer) who will notify you and be able to provide you with a copy of your records to assist you in continuing therapy with a different provider.

Your Personal Data 

What we need 

Evenkeel will be what’s known as the ‘Controller’ of the personal data you provide to us. We collect data about you that includes basic information such as your name, address, email etc. It also includes personal information relevant to your health. With your consent, it may sometimes be helpful to record our meetings. 

Why we need it 

We need to know your basic information in order to maintain your healthcare record. We require personal information in order to provide a therapy service. Recording of sessions are used in clinical supervision, which is a process for ensuring the highest possible standard of therapy is delivered. We will not collect any personal data from you we do not need in order to provide and oversee this service. 

What we do with it 

All the personal data we process is processed by our staff in the UK; however, for the purposes of IT hosting and maintenance, this information is located on servers within the European Union. We use an electronic health record-keeping system, an encrypted email service and Microsoft.  If you have been referred by a 3rd party, then information is shared with them, and we seek your consent for this to happen at the first meeting. This information is usually in the form of a written report but sometimes includes a copy of clinical notes. On occasion, information may be shared with other healthcare professionals. With your consent, recordings of sessions may be shared with a clinical supervisor based within the UK but external to Evenkeel. No other 3rd parties have access to your personal data unless the law allows them to do so or we receive your consent. We may share information without your consent if we are concerned that you are at risk of harm or there is a risk of harm to others.

Therapy is provided via a video conferencing platform using Microsoft TEAMS. As part of a business contingency plan, if TEAMS is not working, then we may be able to switch to telephone for that specific session but then return to TEAMS at the next session. Alternatively, therapy can be provided face-to-face. However, this service is not currently available through Evenkeel, so you will need to discuss with your referrer about accessing therapy from a different provider. Your attendance at an online or telephone appointment implies that you are consenting to access therapy remotely on each and every occasion that you attend.

How long we keep it 

We keep your healthcare record for 10 years. 

What are your rights? 

If at any point you believe the information we process on you is incorrect, you can request to see this information and even have it corrected or deleted. If you wish to raise a complaint about how we have handled your personal data, you can contact our Data Protection Officer, who will investigate the matter. 

If you are not satisfied with our response or believe we are processing your personal data not in accordance with the law, you can complain to the Information Commissioner’s Office (ICO).

If you are making an enquiry about therapy

What we need

When making enquiries about the availability of therapy sessions people provide a range of information that may include their name, address, email etc. It may also include personal information relevant to their health. We cannot control the amount of information provided.

 

Why we need it

 

The information that you provide may help us to advise you about our therapy service. We will not collect any personal data from you we do not need in order to provide and oversee this service.

 

What we do with it

 

We will store any email correspondence. We may also keep a brief record of any communication with you within an electronic medical records system.  All the personal data we process is processed by our staff in the UK however, for the purposes of IT hosting and maintenance this information is located on servers within the European Union. 

 

How long we keep it

 

We keep a record of your enquiry for 10 years.

 

What are your rights?

 

If at any point you believe the information we process on you is incorrect, you can request to see this information and even have it corrected or deleted. If you wish to raise a complaint on how we have handled your personal data, you can contact our Data Protection Officer who will investigate the matter.

If you are a referring agent

What we need

 

When you make enquiries about a therapy referral, you may provide a range of information that may include personal information relevant to the reason for referral. When you refer a patient, we collect data about them that includes basic information such as their name, address, email etc. It also includes personal information relevant to their health and details about you as the referrer, such as your contact details and our agreed fee structure.

 

Why we need it

 

We need to know information about the referred patient in order to create a healthcare record. We require information about your organisation in order to provide a therapy service. We will not collect any personal data we do not need in order to provide and oversee this service.

 

What we do with it

 

All the data we process is processed by our staff in the UK however, for the purposes of IT hosting and maintenance this information is located on servers within the European Union. We use your information to contact you and to raise invoices. We use the patient information to raise a medical record and to contact the patient to arrange an appointment.

 

How long we keep it

 

We keep records and emails for 10 years.

 

What are your rights?

 

If at any point you believe the information we process is incorrect, you can request to see this information and even have it corrected or deleted. If you wish to raise a complaint on how we have handled your personal data, you can contact our Data Protection Officer, who will investigate the matter.

 

If you are not satisfied with our response or believe we are processing your personal data not in accordance with the law you can complain to the Information Commissioner’s Office (ICO).

If you are a student or a supervisee

What we need

 

When you access teaching or supervision we may collect basic information such as your name, address, email etc. It may also include personal information relevant to you as a student or a clinical case.

 

Why we need it

 

We require this information in order to provide teaching or supervision. We will not collect any personal data we do not need in order to provide and oversee this service.

 

What we do with it

 

All the data we process is processed by our staff in the UK however for the purposes of IT hosting and maintenance, this information is located on servers within the European Union. We may use your information to contact you, to seek feedback and to raise invoices. We use keep sensitive clinical information within a supervision record.

 

How long we keep it

 

We keep records and emails for 10 years.

 

What are your rights?

 

If at any point you believe the information we process is incorrect you can request to see this information and even have it corrected or deleted. If you wish to raise a complaint on how we have handled your personal data, you can contact our Data Protection Officer who will investigate the matter.
